JOB DESCRIPTION
To assist in the provision of a comprehensive, high quality and efficient pharmacy service to the Prison Healthcare. The post holder will work as an effective member of the pharmacy team providing comprehensive pharmaceutical services, including Medicines Management and dispensary duties.
Main Responsibilities
1. Clinical/Professional/Specific Managerial Responsibilities
• Ensure the appropriate interpretation of prescriptions prior to their preparation in the pharmacy.
• Provide accurate and appropriate medicines information in response to internal enquiries, including patient and healthcare worker enquiries, utilising all available resources.
• Accurately prepare extemporaneous preparations utilising appropriate equipment whilst ensuring adherence to all relevant good manufacturing guidelines and COSHH.
• Provide a prescription monitoring service including identification of adverse drug reactions, drug related events, drug interactions and rationalisation of medicines.
• Provide an accurate final check on dispensed medication within the pharmacy.
• Obtain a comprehensive and accurate admission medication history at committal stage to ensure that patients receive the correct and appropriate medicine in a timely manner.
• Assess and manage patient’s own drugs to always ensure effective prescribing and patient safety.
• Assist in the development of pharmaceutical care plans in accordance with patients’ diagnosis and assess treatment options available.
• Provide medication counselling and education to patients. This includes the provision of written or verbal information to patients with difficult behaviour or those with impaired cognitive function or impaired senses.
• Provide medicine related information and advice of a therapeutic, legal or procedural nature to prescribers and other healthcare staff always ensuring effective prescribing and patient safety.
• Assist in the collection and analysis of workload data to identify trends in prescribing patterns and encourage cost efficiency.
ESSENTIAL CRITERIA

Experience
Qualifications/ Registration MPharm or equivalent.
Completed and passed pre-registration year and professional exam.
Member of the Pharmaceutical Society of Northern Ireland or eligible for membership.
Hold a current full driving licence which is valid for use in the UK and have access to a car on appointment.
These criteria will be waived in the case of applicants whose disability prohibits driving but who have access to a form of transport approved by the Trust which will permit them to carry out the duties of the post
